Cardio Consult app for iPhone and iPad

Hello Physician,

This app is called “Cardio Consult” and is designed to be used by physicians to communicate with one another regarding patient-specific details accurately and quickly and for referring physicians to more directly contact "Heart Institute of Colorado" physicians. It stands to reason that if using cardio consult makes it very easy to contact "Heart Institute of Colorado" physicians and offices, we can generate more referrals and patients. Cardio consult contains important information for our cardiologists and referring physicians, including call schedule, easy direct physician contact and for the provider contains commonly asked ‘standard of care’ informational links.

The requirements for downloading the application are as follows:
1) Your Phone must be password protected
2) The Application is only compatible with iPhones as of now
3) The Software Version of your iPhone must be IOS 8 or above
4 ) Before using the app please take the survey listed: http://goo.gl/forms/oWq2xABNHY
5) Open the application link on your iPhone only and not on your computer or PC.

Upon opening the app please create an account, click the signup tab. Put a Username, Email, and Password and confirm the terms of agreement. After doing so you will be prompted to confirm your email, the email you used to create the account. After confirming your link, check your junk box, login into the application and enjoy!
